Regulations Regarding Employee CardsIssued pursuant
to 1. The Regulation prescribes the types of commercial activities where the employer has the obligation, upon entering into an employment contract, to issue an employee card to an employee, the information to be included in the employee card and the procedures for issuing such card. 2. The employer has the obligation to issue the employee card to the employee who is employed in the following areas: 2.1. forestry and logging, if the work is carried out in a forest, except for the logging truck drivers engaged in timber transportation; 2.2. construction, if the work is carried out at the object of construction. 3. The employer shall ensure that the employee card is issued to the employee prior to the commencement of the works referred to in Paragraph 2 of this Regulation. 4. The following information shall be included in the employee card: 4.1. given name and surname of the employee; 4.2. photograph or a digital image of the employee; 4.3. personal identity number (if there is no personal identity number - date, month, year of birth) or the number of a visa or a residence permit, attesting that a foreigner has been granted the right to employment, if such entry is required according to the laws and regulations; 4.4. date of entering into the employment contract and the number thereof; 4.5. name (firm) and registration number of the employer or given name, surname and personal identity number of the employer, if the employer is a natural person; 4.6. date of issue of the employee card; 4.7. other information considered relevant by the employer. 5. The employer or the person authorised by the employer has the obligation to sign the employee card. 6. The employer shall keep records of the issued employee cards. 7. The employee has the obligation to keep the employee card at the place of work. 8. The employee must notify the employer immediately, if the employee card: 8.1. has been lost or stolen; 8.2. has become invalid for use or personal data have changed. In such case the employee card must be returned to the employer. 9. The employer shall, after having received the information referred to in Paragraph 8 of this Regulation, register immediately the fact that the employee card has been lost, stolen, has become invalid for use or personal data have changed, and such information shall be kept until a new employee card is made. The employer has the obligation to immediately, but not later than within five working days after the registration of the abovementioned facts, make a new employee card and issue it to the employee. 10. Employee cards, which have been issued until the day of coming into force of this Regulation, shall be valid and can be used also after the coming into force of this Regulation.
